- First application was filed in the civil hospital, Ahmednagar, in the name of Puja Diliprao Khedkar, a resident of Bhalgaon Taluka Pathardi, Ahmednagar district
- She was issued a disability certificate with signatures of three members with 40%disability
JAN 18, 2021
- Second application was filed in the
Ahmednagar civil hospital with same name and address
- She was examined for psychiatric illness and underwent a test called IDEAS (Indian Disability and Evaluation Assessment Scale). She was found suffering from depression
- According to UDID, she was issued a certificate on January 19, 2021, which shows low vision in her both eyes (40% disability), mental illness, brain depression (20% disability), and autogenerated disability (51%)
AUG 23, 2022
- Third application was filed for locomotor disability at YCM hospital, Pimpri, in the name of Puja Manorama Dilip Khedkar
- Her residence was shown as plot no. 53, DehuAlandi Road, Talawade, Pimpri Chinchwad
- A certificate was issued in one day on August 24, 2022, for locomotor disability with diagnosis mentioned as old ACL tear with left knee instability
- She has 7% permanent disability in relation to the left lower limb as per guidelines under the RPwD Act, 2016. A unique disability ID was issued
- At least 40% disability is required to seek a job or exam concession

- Her residence was shown as National Housing Society, Haveli taluka
- The application was autorejected on October 11, 2022, on grounds of duplication
